Lauderdale County Resource Connection HUB: HUB Coordinator
Job Summary: The HUB Coordinator will be the central figure in the day-to-day operations of the Lauderdale County Resource Connection HUB. This role is responsible for leading the implementation of the HUB's mission to connect underserved community members to essential health and social services, unify county resources, and address social determinants of health. The Coordinator will manage the care coordination pathways, oversee the referral platform, and ensure comprehensive support is provided to participants, with a strong focus on improving health outcomes for low-income, minority, and rural populations.

Job Title: Part-Time Community Outreach & Support Specialist
Job Summary: The Community Outreach & Support Specialist will play a vital role in connecting directly with Lauderdale County residents, particularly those in low-income, minority, underserved, and rural areas, to facilitate their access to the Lauderdale County Resource Connection HUB's services. This position will assist the HUB Coordinator in engaging participants, conducting initial assessments, providing referrals, and following up to ensure their needs related to health and social determinants are met. Hours expected 1:00-6:00 pm
